Uploading Documents to Patient Charts

Add supporting documents to enhance AI accuracy and create more comprehensive progress notes.

You must link a patient to your session before uploading documents. Uploading without a linked patient means documents won’t appear or be accessible.

Upload options

From the patient chart: Navigate to the patient’s chart and go to the Documents section.

During a recording session: Link your patient first, then upload documents using the Recorder. Documents must be uploaded before starting the recording.

Using previously uploaded documents

When writing a note, uploading audio, or starting a new recording session, you can select documents already in the patient’s chart. The patient must be linked to access these files.

How documents improve your notes

Documents give the AI additional context for more accurate, detailed progress notes. Consider uploading:

  • Lab work and test results
  • Intake forms
  • Previous medical records
  • Imaging reports
  • Specialist consultations

File requirements and credit usage

  • PDF files only, maximum 25MB
  • Each uploaded document uses 0.25 additional note credits
  • Documents over 10 pages use an additional 0.25 credits per extra page
  • Credit terms may vary if your clinic has a specific agreement with us
